Your situation isn’t limited to a single moment. It’s a series of changes. Frequent test results, new specialists, recurring decisions. You want a physician you can reach whenever something comes up, who already knows your story, and who responds quickly.

Enhanced Support

For a critical stretch

You’re in a period where things are unfolding fast. A hospitalization, a new treatment, a decision that can’t wait, all happening at once. You want a physician closely involved, helping you anticipate what’s next and staying connected throughout.

Where every patient begins. A thorough conversation, focused entirely on you.

A Detailed Consultation is a full 60-minute conversation by video or phone, focused entirely on your situation. No rushed appointment. No agenda but yours. Ask whatever has been on your mind, think out loud, share your thoughts openly. You decide who is in the meeting. Trusted family or friends are welcome to join. If you prefer to speak with Dr. Meadows privately, that is fine too.

Every patient begins here. It is how Dr. Meadows comes to understand your situation. Many return for additional consultations later, when a treatment plan changes, a hospitalization happens, or they simply want time to think something through carefully.

For many people, one conversation is all they need. Others choose to stay connected through Ongoing Support. There is no pressure to continue beyond your consultation.

*Patients enrolled in Ongoing or Enhanced Support pay $350 for Detailed Consultations.

What’s Included

60-minute conversation by video or phone

Dr. Meadows sets aside the full hour so there is time to address what is on your mind without feeling rushed.

Pre-consultation record review

You can share medical records, test results, or notes in advance. Dr. Meadows reviews them before your conversation so your time together is thorough from the start. Optional but encouraged.

Follow-up secure messaging

After your consultation, you can message Dr. Meadows through the next business day with any follow-up questions.

Family or trusted friends welcome

You decide who participates. They can join with you or from their own location. Only with your consent.

Written summary delivered after your conversation

Dr. Meadows prepares a written record of what was discussed. It is delivered to your secure portal and is yours to keep, share with family, or bring to your other physicians.

Most pressing concerns can be addressed meaningfully in a single conversation. If at the end of your consultation you feel there is more to work through, Dr. Meadows will discuss the best path forward for your situation. Additional consultations can be scheduled anytime you wish. There is never pressure to add services you do not want.

That happens often, and it’s ok. These conversations touch on things that matter, and feeling something during them is part of how people process what they’re facing. Dr. Meadows will not rush you, redirect you, or treat emotion as an interruption. Take whatever time you need.

No. You do not need a referral from your doctor or anyone else. PalliMate operates independently of the traditional healthcare system. This is your choice.

The only qualification is that you believe PalliMate can help you. There are no clinical requirements, no specific diagnosis, no required level of illness, and no prior approval process. If you are in a situation that has you confused, frustrated, feeling unsupported, or facing any other unmet need, you are welcome here.

At the end of your consultation, Dr. Meadows will be happy to discuss the best options based on your needs. There is no obligation and no pressure. If you decide you want to continue, enrollment is simple. If you decide one conversation was all you needed, that is fine too. You can always reach out later if you would like to meet with Dr. Meadows again.

Ongoing Support

Expert physician access whenever you need it. Month-to-month membership, as you see fit.

Ongoing Support is a monthly membership for patients who want continued access to Dr. Meadows after their initial consultation. Through your secure portal, you can message him directly about your situation. An important benefit, you can also designate a PalliMate Primary Caregiver who can communicate with Dr. Meadows on your behalf.

The value of Ongoing Support is that you’re connected directly with Dr. Meadows, who already knows your situation. Whenever needed — new test results, an upcoming conversation with another doctor, a worry that’s been weighing on you — you don’t have to wait weeks for answers. Dr. Meadows responds thoughtfully and thoroughly, typically within one business day. Some patients message often. Others only when something specific comes up. Either way, he is there.

Ongoing Support can be started at any time. It requires a Detailed Consultation to be completed within the last 90 days, so Dr. Meadows has a clear understanding of your situation.

What’s Included

Unlimited secure messaging with Dr. Meadows

Send questions, updates, or concerns whenever you like. Dr. Meadows typically responds the same/next business day.

Secure document storage

Your portal allows you to store and share important files with Dr. Meadows. Advance directives, treatment summaries, and care records are easily accessible when needed.

Option to designate a Primary Caregiver

A trusted family member or friend can receive their own portal access and communicate with Dr. Meadows on your behalf. Only one Primary Caregiver may be designated at a time.

Priority scheduling for Detailed Consultations

If your situation calls for a Detailed Consultation, you receive priority access to Dr. Meadows’ schedule. Consultations are billed separately at the member rate.

Dr. Meadows typically responds within one business day, excluding holidays. Because Dr. Meadows replies to patient messages throughout the day, replies can come quickly. It’s important to always remember that secure messaging is not appropriate for urgent situations and should never be used for emergencies.

Anything related to your medical situation, your care, or decisions you are facing. Common reasons patients reach out include questions after an appointment with their other doctors, concerns about a new diagnosis or test result, confusion about a treatment recommendation, preparation for an upcoming appointment, or simply needing help thinking through something that has been weighing on them. No question is too big or too small.

PalliMate is not an emergency service. If you are enrolled in Ongoing Support and have an emergency, you should always seek immediate care through 911 or other appropriate means. Dr. Meadows remains available to help you sort through any new or existing concerns that changed as a result of your emergency, but you should never delay urgent care.

Some situations call for real-time conversation. If a follow-up Detailed Consultation is necessary, you will receive priority scheduling at the member rate (potentially same day). If you are going through a particularly intense or rapidly changing period, he may also recommend Enhanced Support, which adds proactive check-ins and brief phone calls to your membership. You will never be pressured to add services you do not want.

Enhanced Support is the most active level of support PalliMate offers. It is a 30-day program designed for patients navigating a critical or rapidly changing period in their care. This might include a recent hospital discharge, the start of a new treatment, a major care transition, difficult imminent care decisions, or any reason closer involvement from Dr. Meadows would be helpful.

Unlike Ongoing Support, where most conversations begin when you reach out, Enhanced Support is more proactive. Dr. Meadows regularly checks in throughout the 30-day period, anticipates challenges, and stays closely engaged as your situation develops. As always, you can also reach out whenever you want.

When messaging is not enough, Dr. Meadows may initiate brief, focused phone calls to address specific issues in real time. These calls are included in the Enhanced Support fee and are intended to supplement messaging, not replace a Detailed Consultation if needed.

Enhanced Support is an add-on layer to Ongoing Support and runs for 30 consecutive days. It does not automatically renew. At the end of the period, your care level returns to your Ongoing Support membership. You can repeat Enhanced Support as often as you like (subject to availability).

Proactive check-ins and ongoing conversation

Dr. Meadows checks in regularly and remains proactively engaged during the 30-day period.

Brief, focused phone calls when needed

When needed, Dr. Meadows may initiate brief phone calls to discuss things. These calls are included in the Enhanced Support fee.

More active communication with your healthcare team

With your consent, Dr. Meadows may communicate more frequently with your other physicians and care providers.

Highest priority scheduling for Detailed Consultations

When a longer conversation is needed, Enhanced Support members receive the highest scheduling priority available (sometimes same day). Consultations are billed separately at the member rate.

Deeper advance care planning and goal-setting support

During this time, Dr. Meadows will work closely with you to clarify your goals of care, discuss treatment preferences, and help you document those decisions.

— Enhanced Support —

No. Enhanced Support is an add-on to an active Ongoing Support membership. If you are a new patient and would like Enhanced Support from the beginning, you will be enrolled in both services (Ongoing & Enhanced Support) at the same time and complete your initial Detailed Consultation at the start of care. Dr. Meadows will coordinate these steps with you.

Both services provide great support but each one provides a different style of engagement. Both give you key features like unlimited messaging and Primary Caregiver access, but Enhanced Support allows Dr. Meadows to be more proactive in your communication. With Enhanced Support you can expect frequent check-ins to see how you are doing, as well as occasional calls if a quick conversation is needed. In addition, Enhanced Support provides more “behind the scenes” activity, such as more engagement with your other doctors or care teams.

Enhanced Support does not auto-renew. At the end of its 30 days, the service automatically stops. However, your underlying Ongoing Support membership remains fully intact. This means you continue to have unlimited messaging access with Dr. Meadows, as well as all other services provided by Ongoing Support. This transition is seamless and does not require any action on your end.

You may re-enroll in Enhanced Support at any time, subject to availability.

Enhanced Support can be very helpful, especially when unexpected things are happening such as a hospitalization, a sudden change in your care plan, or enrollment in a home-based care service. During these times, Enhanced Support remains fully available to you.

The decision to enroll in Enhanced Support is yours alone. You do not have to qualify, have a certain diagnosis, have a poor prognosis, or jump through any authorization hoops. If you want Enhanced Support, you can request to be enrolled.

Detailed Consultation

Payment is required when you book your appointment. This secures your meeting time. Once it’s scheduled, you’ll receive everything you need to connect with Dr. Meadows.

If you cancel more than 24 hours in advance, you can receive a full refund or reschedule at no cost. With less than 24 hours notice, you may reschedule once, but the fee is not refundable. Any missed appointments without prior notice are not refunded or rescheduled.

Ongoing Support

Ongoing Support can be started any time. The first month is prorated from your enrollment date through the end of that calendar month. After that, Ongoing Support automatically renews on the first of each month. A valid payment method needs to remain on file during your enrolled.

You can cancel any time by sending Dr. Meadows a secure message. No explanation is needed. Your membership stays active through the end of the current paid month. You will not be charged after that. Prorated refunds are not provided.

Enhanced Support

Enhanced Support can be added to your Ongoing Support membership at any time. You are billed separately and full payment is required to begin.

Enhanced Support starts at the time of purchase and runs for 30 consecutive days. Enhanced Support does not automatically renew and is not refundable once purchased.

Additional 30-day periods can be added as needed. Because total enrollment is limited, current Enhanced Support patients have first right to renew before additional enrollments are accepted.

Payment Information

Private Pay

PalliMate does not accept insurance and will not submit claims on your behalf. This is intentional. Insurance reimbursement offers little or no coverage for the kind of work PalliMate does, and the policies that do offer limited coverage often restrict the kind of care you can receive. Working outside that system means Dr. Meadows can focus entirely on you, with no outside incentives, no billing confusion, and no institutional priorities shaping the conversation.

All PalliMate services may be paid with a credit card, debit card, HSA or FSA card. Please confirm HSA or FSA eligibility with your account administrator.

Medicare

PalliMate is available to all Medicare beneficiaries, but all fees must be paid personally. Because PalliMate is a private service, Medicare patients are required to sign a Medicare Private Contract. PalliMate will provide you with this standard form before your service begins.

This will not affect your Medicare coverage in any way, nor will it affect any other doctors, hospitals, or services you access through Medicare. This form is simply a requirement to confirm that you understand you are personally responsible for the fees.

Understanding The Scope

What PalliMate doesn't do.

A quality practice means honest limits. The list below is what falls outside PalliMate’s scope, so you know before you begin.

Emergency & Urgent Care

PalliMate is not an emergency service. If something feels urgent or life-threatening, call 911 or go to the nearest emergency department. Secure messaging is not the right channel for time-sensitive concerns and is not monitored in real time.

Diagnose, Prescribe, or Manage Treatment

Through PalliMate, Dr. Meadows does not diagnose conditions, prescribe medications, order tests, or manage your medical treatment. Those responsibilities stay with your other doctors.

Care Coordination & Logistics

PalliMate does not schedule appointments with your other doctors, arrange transportation, or handle the day-to-day logistics of your care. Those tasks are typically handled by your primary doctor, case managers, navigation coordinators, and caregivers.

Other Professional Services

PalliMate does not provide psychotherapy, mental health counseling, financial planning, or insurance guidance. Conversations with Dr. Meadows may give you helpful clarity in these areas, but if formal services are appropriate, he will help you think through next steps.

These limits assure Dr. Meadows’ attention remains focused where it belongs: on helping you understand your diagnosis, treatments, and options over time.
