The Conversation
A full 60-minute conversation where Dr. Meadows comes to understand your situation. It’s where every patient begins, and the foundation everything else is built on. For many people, one conversation is all they need. Others meet frequently.
Staying Connected
Unlimited messaging with the physician who already knows your story, so answers are quick and insightful. You can also give a trusted caregiver their own access to communicate with Dr. Meadows on your behalf.
Staying Close
Dr. Meadows stays proactively involved through a fast-moving stretch, with regular check-ins and brief phone calls when something is better handled in real time. For the moments you simply need more.

The Foundation
Where every patient begins. A thorough conversation, focused entirely on you.
A Detailed Consultation is a full 60-minute conversation by video or phone, focused entirely on your situation. No rushed appointment. No agenda but yours. Ask whatever has been on your mind, think out loud, share your thoughts openly. You decide who is in the meeting. Trusted family or friends are welcome to join. If you prefer to speak with Dr. Meadows privately, that is fine too.
This is where PalliMate is different. As a physician, Dr. Meadows can go into the real detail of your condition, your test results, your medications, and the trade-offs between your options, in plain language and with the time to actually explain it. But the conversation is never only clinical. It’s also about what all of it means for your life, what worries you, and what matters most to you. Few people get both in the same conversation. That combination is the heart of what PalliMate offers.
Every patient begins here. It is how Dr. Meadows comes to understand your situation. Many return for additional consultations later, when a treatment plan changes, a hospitalization happens, or they simply want time to think something through carefully.
For some people, one conversation is all they need. Others choose to stay connected through Ongoing or Enhanced Support. There is no pressure to continue beyond your consultation.

The full hour is set aside so there is time to address what is on your mind without feeling rushed.
Pre-consultation record review
You can share medical records, test results, or notes in advance. Dr. Meadows reviews them before your conversation so your time together is thorough from the start. Optional but encouraged.
Follow-up secure messaging
After your consultation, you can message Dr. Meadows through the next business day with any follow-up questions.
Family or trusted friends welcome
You decide who participates. They can join with you or from their own location. Only with your consent.
Written summary delivered after your conversation
Dr. Meadows prepares a written record of what was discussed. It is delivered to your secure portal and is yours to keep, share with family, or bring to your other physicians.

No. You do not need a list of questions, organized records, or a clear sense of what to ask. Many people arrive unsure where to begin, and helping you find the right questions is part of Dr. Meadows’ role. If you would like to share medical records or test results in advance, you are welcome to, and it can make the conversation more thorough, but it is entirely optional. Come as you are.
Most pressing concerns can be addressed meaningfully in a single conversation. If at the end of your consultation you feel there is more to work through, Dr. Meadows will discuss the best path forward for your situation. Additional consultations can be scheduled anytime you wish. There is never pressure to add services you do not want.
That happens often, and it’s okay. These conversations touch on things that matter, and feeling something during them is part of how people process what they’re facing. Dr. Meadows will not rush you, redirect you, or treat emotion as an interruption. Take whatever time you need.
At the end of your consultation, Dr. Meadows will be happy to discuss the best options based on your needs. There is no obligation and no pressure. If you decide you want to continue with Ongoing or Enhanced Support, enrollment is simple. If you decide one conversation was all you needed, that is fine too. You can always reach out later if you would like to meet with Dr. Meadows again.

Ongoing Support is a monthly membership for patients who want to stay connected to Dr. Meadows after their initial consultation. Through your secure portal, you can message him directly, whenever a question comes up.
This fills a gap most people know well. When something is worrying you, the usual options are the internet, which knows nothing about you, or a call to your doctor’s office where you leave a message and wait for someone to call you back. Ongoing Support is neither. Your question goes straight to a physician who already knows your story, and the answer is about your situation specifically. You can even upload a new test result or record and ask Dr. Meadows directly for feedback. Not every question needs a full 60-minute conversation. Sometimes you just need a trusted answer, quickly. Ongoing Support is for all the moments in between.
Some patients message often. Others only when something specific comes up. Either way, Dr. Meadows responds thoughtfully and thoroughly, typically within one business day and often sooner.
Ongoing Support also gives you one of PalliMate’s most valued features. You can designate a trusted family member or friend as your Primary Caregiver, giving them their own portal access to communicate with Dr. Meadows on your behalf. For many patients, having a caregiver stay in the loop and ask questions directly is a relief of its own.
Ongoing Support can be started at any time. It requires a Detailed Consultation to be completed within the last 90 days, so Dr. Meadows has a clear understanding of your situation.

Your portal allows you to store and share important files with Dr. Meadows. Advance directives, treatment summaries, and care records are easily accessible when needed.
Option to designate a Primary Caregiver
A trusted family member or friend can receive their own portal access and communicate with Dr. Meadows on your behalf. Only one Primary Caregiver may be designated at a time.
Priority scheduling for Detailed Consultations
If your situation calls for a follow-up Detailed Consultation, you receive priority access to Dr. Meadows’ schedule. Consultations are billed separately at the member rate.

Dr. Meadows typically responds within one business day, excluding holidays, and often sooner. Because he replies to patient messages throughout the day, you usually will not be waiting long. It is important to remember that secure messaging is not appropriate for urgent situations and should never be used for emergencies.
Anything related to your medical situation, your care, or decisions you are facing. Common reasons patients reach out include questions after an appointment with their other doctors, concerns about a new diagnosis or test result, confusion about a treatment recommendation, preparation for an upcoming appointment, or simply needing help thinking through something that has been weighing on them. No question is too big or too small.
PalliMate is not an emergency service. If you have an emergency, always seek immediate care through 911 or other appropriate means. Once you are safe, Dr. Meadows remains available to help you make sense of what happened and what it means going forward, but you should never delay urgent care to send a message.
Some situations call for real-time conversation. If a follow-up Detailed Consultation would help, you receive priority scheduling at the member rate. And if you are going through a particularly intense or rapidly changing period, Dr. Meadows may suggest Enhanced Support, which adds proactive check-ins and brief phone calls to your membership. You will never be pressured to add services you do not want.
Proactive Engagement
Intensive, proactive support during critical or rapidly changing times.
Enhanced Support is the most active level of support PalliMate offers. It is a 30-day program for patients navigating a critical or rapidly changing period in their care, such as a recent hospital discharge, the start of a new treatment, a major care transition, or a series of difficult decisions.
Unlike Ongoing Support, where most conversations begin when you reach out, Enhanced Support is more proactive. Dr. Meadows checks in regularly, anticipates what’s coming, and stays closely engaged as your situation moves forward. And when a message isn’t enough, he may initiate a brief phone call to work through something in real time.
This is also when Dr. Meadows can engage more directly with the rest of your care team. With your consent, he may speak with your other physicians and providers to clarify a recommendation, gather context, or make sure your goals are being heard. During an intense stretch, having a physician who can reach into the system on your behalf is often where Enhanced Support proves most valuable.
Enhanced Support is added to an active Ongoing Support membership and runs for 30 consecutive days. It does not automatically renew. At the end of the period, your care returns to your Ongoing Support membership. You can repeat Enhanced Support as often as you like, subject to availability.

Brief, focused phone calls when needed
When a message isn’t enough, Dr. Meadows may initiate a brief phone call to talk something through. These calls are included.
More active communication with your care team
With your consent, Dr. Meadows may speak more often with your other physicians and providers, helping clarify recommendations and make sure your goals are being heard.
Highest priority scheduling for follow-up Detailed Consultations
When a longer conversation is needed, Enhanced Support members receive the highest scheduling priority available. Consultations are billed separately at the member rate.
Deeper advance care planning and goal-setting support
If it fits your situation, Dr. Meadows can work closely with you to clarify what matters most, think through treatment preferences, and put your wishes into words you can share with your family and other doctors.

Enhanced Support adds a higher level of engagement on top of everything Ongoing Support already includes. Dr. Meadows stays proactively involved rather than waiting for you to reach out, checking in regularly throughout the period. He may pick up the phone when something is better handled by voice, and with your consent he stays in closer contact with your other doctors. It is the right fit when your situation calls for more hands-on involvement than messaging alone provides.
The decision is yours alone. There is no diagnosis you must have, no prognosis you must meet, and no approval to obtain. If you are in a period where you want Dr. Meadows more closely involved, you can request it.
Yes. Enhanced Support works alongside an Ongoing Support membership, so if you want it from the start, you simply enroll in both and complete your initial Detailed Consultation at the beginning of care. Dr. Meadows will coordinate these steps with you. This is an option many patients choose, especially if they are coming to PalliMate during a difficult time.
Enhanced Support does not auto-renew. At the end of the 30 days it simply stops, and your underlying Ongoing Support membership continues exactly as before, including unlimited messaging and all its other features. The transition is seamless and requires nothing from you. You may re-enroll in Enhanced Support at any time, subject to availability.
Enhanced Support is built for supporting unpredictable moments. A hospitalization, a move to hospice, or a sudden decline does not interrupt your service. In fact, these are often the times the service matters most, and Dr. Meadows stays with you through them. If you are ever unable to reach out yourself, your designated Primary Caregiver can communicate with Dr. Meadows on your behalf, so support continues without interruption. Because Enhanced Support reserves dedicated time for you, it runs for the full 30 days and is not refundable once it begins.
Pricing & Billing

Payment is required when you book, which secures your meeting time. Once it’s scheduled, you’ll receive everything you need to connect with Dr. Meadows.
Plans change, and rescheduling is easy. Cancel or reschedule more than 24 hours ahead and you can move your appointment at no cost or receive a full refund. With less than 24 hours’ notice, you can reschedule once, though the fee is no longer refundable. Appointments missed without notice cannot be refunded or rescheduled.
Ongoing Support can be started at any time. Your first month is prorated from your enrollment date through the end of that calendar month, and renews on the first of each month after that. A valid payment method stays on file while you are enrolled.
You can cancel anytime by sending Dr. Meadows a secure message. No explanation is needed. Your membership stays active through the end of the current paid month, with no charges after that. Prorated refunds are not provided.
Enhanced Support can be added to your Ongoing Support membership at any time. It is billed separately, and full payment is required to begin.
Enhanced Support starts at purchase and runs for 30 consecutive days. It does not automatically renew and is not refundable once it begins.
Additional 30-day periods can be added as needed. Because total enrollment is limited, current Enhanced Support patients have first right to renew before new enrollments are accepted.
Payment Information
Private Pay
PalliMate does not accept insurance and will not submit claims on your behalf. This is intentional. Insurance offers little or no coverage for the kind of work PalliMate does, and the policies that do often restrict the support you can receive. Working outside that system means Dr. Meadows can focus entirely on you, with no outside incentives, no billing confusion, and no institutional priorities shaping the conversation.
All PalliMate services may be paid by credit card, debit card, or HSA/FSA card. Please confirm HSA or FSA eligibility with your account administrator.
Medicare
PalliMate is available to all Medicare beneficiaries, though Medicare will not reimburse the fees, so they are paid personally. Because PalliMate is a private service, you will be asked to sign a simple acknowledgment confirming you understand this, a standard form known as a Medicare Private Contract. PalliMate provides it before your service begins.
Signing it changes nothing about your Medicare. Your coverage, your other doctors, your hospital benefits, and every other service you access through Medicare all remain exactly as they are. The form applies only to the care you receive through PalliMate.
Understanding The Scope
A service built on trust means not only understanding what it provides, but being clear about what it won’t. PalliMate intentionally operates within certain limits.
PalliMate isn’t an emergency service, and messages aren’t monitored to provide emergency care. If something feels urgent or life-threatening, you should seek appropriate help right away. Once things are stable, Dr. Meadows is here to help you process what happened and what it means moving forward.
Dr. Meadows uses his medical knowledge to go as deep into your diagnosis, your medications, and your choices as you want to. What he won’t do is prescribe, order tests, or take over your treatment. Those roles stay with your other doctors. It’s part of what lets him be a truly independent voice in your corner, with nothing shaping his guidance but your best interest.
Dr. Meadows won’t hesitate to help you think through available resources, services, and community support. What PalliMate doesn’t handle is the logistics, things like booking appointments, arranging rides, and managing the day-to-day of your care. Your primary doctor, case managers, and caregivers are better positioned for that.
PalliMate isn’t therapy, financial planning, or insurance guidance. Talking with Dr. Meadows often brings clarity in these areas, but when something more formal would help, he’ll point you toward the right person.
