Step 1

Submit the Form

You will receive immediate confirmation when your form is successfully submitted. You will also receive a welcome email from PalliMate to the email address you provided. Check your spam folder if it does not arrive within a few minutes.

Step 2

Dr. Meadows Reviews

Dr. Meadows will personally review your information, typically within one business day. You will then receive a second email with instructions for accessing your secure patient portal.

Step 3

Schedule Your Meeting

Once you have portal access, you and Dr. Meadows will coordinate a time for your meeting. All scheduling and communication happens through the portal.

If you are completing this form on behalf of the patient, please include your contact information in the “YOUR INFO” section as well as the patient’s information in the “CLIENT INFO” section.
