Behind Every Patient
As a caregiver, you are the one keeping track of everything. Sitting in on appointments. Asking the questions that otherwise would not be asked. Catching what gets missed and following up on what gets forgotten.
It is quiet, often invisible work, and very little of it would happen without you.
That role takes a real toll, even when carried willingly and out of love. Beyond the physical and logistical demands, there is the emotional weight of watching someone you love suffer while feeling powerless to change it.
PalliMate cannot remove the demands of caregiving. What we can do is help you better understand what is happening and what comes next. When patients see the path more clearly, the people walking beside them can too.

The Primary Caregiver Role
Having someone you trust in your corner matters. Giving them the tools to act on your behalf matters just as much.
That is why patients enrolled in Ongoing or Enhanced Support may designate a qualifying person as their PalliMate Primary Caregiver. With the patient’s consent, that person receives their own portal access and can communicate directly with Dr. Meadows on the patient’s behalf.
A Primary Caregiver can send secure messages, upload and review documents, and schedule PalliMate services for the patient. And while PalliMate patients must reside in Tennessee, the Primary Caregiver can live anywhere in the U.S.
This optional designation is the patient’s to make, and the role exists to help the patient receive all PalliMate has to offer.
Primary Caregiver Questions
The process begins with the patient’s consent. They send Dr. Meadows a secure message indicating they would like to designate a Primary Caregiver and provide that person’s name, contact information, and relationship. The prospective Primary Caregiver will be asked to sign an acknowledgement explaining the role, its purpose, and its limits. Once that step is complete, they can communicate with Dr. Meadows directly and access the patient’s portal within the scope of their role.
A Primary Caregiver can be any trusted adult the patient chooses — a spouse, adult child, sibling, close friend, or anyone else the patient feels confident in. They must be 18 or older and may live anywhere in the United States. They do not need to hold legal power of attorney, though many patients choose the same person for both roles. The decision rests entirely with the patient.
The PalliMate Primary Caregiver designation is a communication role, not a legal one. It gives a trusted person separate access to the patient’s portal and the ability to communicate with Dr. Meadows on the patient’s behalf. It does not establish healthcare power of attorney, legal decision-making authority, or any medical standing.
Additionally, being a PalliMate Primary Caregiver does not create a physician-patient relationship between you and Dr. Meadows or PalliMate.
Patients who wish to establish legal authority for a trusted person should consult an attorney about the appropriate documentation for their situation.
Yes. A patient may change or remove the Primary Caregiver designation at any time. The Primary Caregiver themselves may also choose to step back from the role whenever they wish. Either change is typically processed within one business day. The arrangement is flexible by design and entirely voluntary for both parties.
The Primary Caregiver designation is available with either Ongoing or Enhanced Support. Any patient enrolled in those services can assign a Primary Caregiver. For patients who exclusively use Detailed Consultations to work with Dr. Meadows, they may have trusted family members or friends join their consultations in real time. This can be a valuable way for others to hear the same information firsthand, ask questions, and help the patient coordinate with Dr. Meadows. But the features of the Primary Caregiver role (portal access, secure messaging, uploading documentation, etc.) are only available with Ongoing or Enhanced Support.
