Here is what to expect.

Step 1

Submit the Form

You will receive a confirmation email at the address you provide letting you know the form was sent. The note comes from Dr. Meadows directly. Check your spam folder if it does not arrive within a few minutes.

Step 2

Dr. Meadows Replies

Once submitted, Dr. Meadows will personally review your information, typically within one business day. You will then receive a second email with instructions for accessing your secure patient portal.

Step 3

Schedule Your Meeting

Once you have portal access, you and Dr. Meadows will coordinate a time for your meeting. All scheduling and communication happens through the portal, directly with him.

If you are completing this form on behalf of the patient, please include your contact information in the “YOUR INFO” section as well as the patient’s information in the “CLIENT INFO” section.
